All students must follow all deadlines and instructions found in the Arbiter Sports site regarding physical examinations and waiver forms. Physical Exam forms should be submitted to the nurse only.
PHYSICAL EXAM FORMS SHOULD NEVER BE SUBMITTED TO COACHES AND/OR BOOSTER CLUBS.

The Health History Updated Questionnaire must be submitted to the nurse prior to the start of each sport season a student will participate in (FALL, WINTER and/or SPRING).
